As of October 1, 2013, the Voter Information Verification Act (VIVA - HB589) enacted by the North Carolina General Assembly, provides a form to be completed by a near relative* or personal representative of a deceased voter to request removal of voter from the voter registration records.
* A near relative is defined by the North Carolina General Statutes as: a spouse, brother, sister, parent, grandparent, child, grandchild, mother-in-law, father-in-law, daughter-in-law, son-in-law, stepparent or stepchild.

Print the form out and complete as thoroughly as possible. Return completed forms either in person, or by mail, fax or scan & email to:
